I don't have a vacuum nipple on the intake manifold, so do I just cap off the connection on the fresh air tube?

What are you doing?
You have two vacuum nipples on the far driver's side of the intake manifold pointing forward. My 1994 855T has no nipple on the far passenger side of the intake.

Replacing the hose going from the oil separator to the fresh air tube specifically. I was confused because the hose I took off had the vacuum fitting on the fresh air tube, but there's no tube coming out the other side of the "padding" where the oil separator is.
On a side note, the sludge inside the PCV tube was revolting. I should have taken a picture

On my 1994 855T, the hose going from my Oil Separator to the PTC is a relatively large diameter hose with no other smaller vacuum hoses bundled with it. My replacement hose was more of an "assembly", with a small vacuum line running within the padding of the large diameter hose and extending about 6" from each end. I cut off the ends of this small diameter vacuum hose since my OEM hose didn't use them.
