Thinking of buying a 2006 s60r 5 speed with over 200k miles. Would this be crazy?

Not really.
Of course you need to take in consideration condition of the vehicle and service records (which should be extensive) and compare against the price.

^ What he said. But with an average of 40,000 miles per year, I bet most of those miles are highway, which is easier on everything than constant city traffic.

If it's a good price then I don't see why not. I am trying to get my parents to buy an 2010 TDI with 140K on her, now that is high miles for the year!
The only two items I would worry about are the dampeners since those are expensive (about $400 each IIRC) and the bevel gear to tranny splines.

Not bad, just keep in mind about the suspension as that's the only real major cost aside from the AWD. Also make sure the timing belt has been done for a second time as it's due at 210K depending on when it was last done. Not expensive or hard to do but something to consider...
